JOB TITLE: Family Support Workers (Bilingual - Spanish/English)

Hours: Full-Time  Monday – Friday

Job Description:

Provides family-centered, strength-based interventions directed at establishing a trusting relationship within families to optimize parent/child relationships.

Key Responsibilities:

  • Establishes and maintains excellent relationships with pregnant women and families enrolled in the program.
  • Provides a range of culturally competent home based services on a weekly basis for all families enrolled in the program through a combination of in-person and virtual visitation.
  • Establishes trusting, non-judgmental relationships with families and treats families equitably and fairly.
  • Provides prenatal and other health education and assists in strengthening the parent-child relationship.
  • Assists parents in improving their skills to optimize the home environment for child’s well-being and safety and establishes goals and a plan for accomplishing those goals.
  • Identifies and makes referrals to other support services in the community.
  • Assists in making and attending health and human service appointments, including activities related to educational and employment goals.
  • Completes all assessments and evaluation paperwork within program guidelines and timelines.
  • Maintains clients’ records and completes all necessary documentation with program timelines.
  • Contacts referrals in a timely manner and maintains weekly visits with all clients in accordance with the program requirements.
  • Participates in case conference and collaborates with other service providers working with the families.
  • Attends staff meetings to insure excellent communication among all program staff and high quality outcomes for children and families.
  • Constructively utilizes formal and peer supervision to evaluate and improve effectiveness in providing services to clients, including active participation in supervision, team meetings and agency meetings.
  • Participates in required trainings in the Home Visiting Network.
  • Performs data entry as necessary.
  • Facilitates and engages families in group activities and other program events.
  • Attends Healthy Families America trainings, and other ongoing professional development as appropriate.
  • Engages the residents of the community with the Healthy Families program.
  • Models the agency values of respect, compassion, integrity, inclusivity and excellence in all activities.
  • Uses and discloses PHI only as authorized, as necessary to carry out job duties
  • Completes and maintains privacy and security training
  • Reports suspected violations including those of a business associate


Successful candidates will have:

  • Bachelor’s Degree in Human Services, Child Development or a related field with a minimum of three years of public health or community development experience.
  • Complete BCI and fingerprints
  • Valid driver’s license and registered/inspected vehicle – Required
  • Valid CPR and First Aid Training.
